Application Procedure:
Informal inquiries regarding the program and intent to submit an application should be directed to the Regional Office serving the State in which the applicant is located. The standard application forms, as furnished by PHS and required by 45 CFR 92 for State and local governments, must be used for this program. Application is made by the submission of the standard PHS application. Application kits are obtained by writing to the HRSA Grants Application Center, 901 Russell Avenue, Suite 450, Gaithersburg, MD 20879. This program is subject to the provisions of 45 CFR for State and local governments and 45 CFR 74 for nonprofit organizations.
Note: Each program will indicate whether applications are to be submitted to the Federal headquarters, regional or local office, or to a State or local government office.
Award Procedure:
Applications are reviewed by the the Bureau of Health Professions and the Bureau of Primary Health Care. Applications are reviewed for merit and are recommended for approval or disapproval. Final decisions are made jointly by the Associate Administrator for Health Professions and the Associate Administrator for Primary Care.
Note: Grant payments may be made by a letter of credit, advance by Treasury check, or reimbursement by Treasury check.
Awards may be made by the headquarters office directly to the applicant, an agency field office, a regional office,
or by an authorized county office. The assistance may pass through the initial applicant for further distribution by
intermediate level applicants to groups or individuals in the private sector.
